Boarding Home Statement
Stark County Job and Family Services will reimburse licensed agency foster caregivers for board and respite care services. Certified SCJFS caregivers must submit a prescribed agency Boarding Home Statement at the end of each month containing information identified on the Statement. Reimbursement is made based on care provided in the month preceding the actual receipt of reimbursement.
Training Credit for Therapy
The active participation of a foster caregiver in the child's therapy is an important part of the child's healing process. Foster caregivers receive training about a variety of mental health issues and other issues related to foster care through their involvement in the therapy process. Often, as a result of their participation in a child's therapy, foster parents are receiving training on the issues and being dealt with in therapy. To receive training credit for participating in therapy, have the therapist complete the Training Credit for Therapy form.
Training Competency Form for Non OCWTP Workshop
Foster caregivers may attend training that is not a part of the Ohio Child Welfare Training Program. To receive credit for attending these trainings the foster caregiver must complete the Training Competency Form for Non OCWTP Workshops and have the trainer sign the form.
Book/Video/Audio/Internet Training Form
The acceptance of continuing training that is completed outside of a classroom where a trainer is not present shall be considered by the agency. To be accepted by the agency, the training must include a transfer of learning component following the training. The transfer of learning component is satisfied with the completion of the Book, Video Tape, Audio and Internet Workshop Training Form.
Foster Parent Mileage Reimbursement Request Form
Payment of approved transportation expenses will be made to a foster parent. Payment will be made at the existing county approved rate of reimbursement to the provider. Monthly statements must be submitted to the child on the Foster Parent Mileage Reimbursement Request Form.
